From Burnout to Building a Thriving Cash Practice: Kathryn Deaton's Journey with Elite Physical Therapy

Many physical therapists dream about having more freedom.

More autonomy.

More time with family.

More control over patient care.

For Kathryn Deaton, founder of Elite Physical Therapy, that dream became reality after years of working in traditional outpatient settings and realizing the care she wanted to provide no longer aligned with the environment she was practicing in.

In this Rainmaker Spotlight episode, Kathryn shares her journey from corporate outpatient physical therapy to building one of Oklahoma City's leading cash-based practices. Along the way, she discusses entrepreneurship, pricing, leadership, hiring, burnout, and the lessons she learned while creating a business built around her values.

An Entrepreneur Long Before Physical Therapy

Unlike many practice owners who become entrepreneurs after growing frustrated with the healthcare system, Kathryn always knew she wanted to build something of her own.

As a child, she sold handmade bracelets and constantly looked for creative ways to stand out. That entrepreneurial mindset followed her into physical therapy school and eventually into private practice ownership.

After graduating in 2014, earning her orthopedic clinical specialist (OCS) certification, and completing advanced manual therapy training, Kathryn expected to spend her career growing within a private practice environment she loved.

Then a corporate acquisition changed everything.

The clinic culture shifted.

The patient experience changed.

And the career path she thought she wanted suddenly looked very different.

When Burnout Became a Turning Point

Many clinicians describe burnout as being overwhelmed.

Kathryn describes it differently.

She calls it moral injury.

At one point, she found herself juggling multiple patients simultaneously while trying to provide the level of care she knew they deserved.

The experience forced her to ask a difficult question:

"Is this really how physical therapy is supposed to be delivered?"

She realized the issue wasn't that she disliked physical therapy.

She disliked being forced to practice in a way that conflicted with her values.

That realization ultimately became the catalyst for starting her own business.

Starting Elite Physical Therapy

While on maternity leave with her second daughter, something unexpected happened.

Former patients started reaching out.

They wanted to continue working with her.

They trusted her.

And many were willing to pay out of pocket to do so.

Initially, Kathryn treated patients from her home before transitioning into a concierge model where she traveled throughout the Oklahoma City area providing one-on-one care.

The demand was immediate.

Patients appreciated the personalized experience.

Referrals continued to grow.

And Elite Physical Therapy was born.

The Hidden Challenge of Concierge Care

From a financial standpoint, concierge care was extremely attractive.

Profit margins approached 95%.

Overhead was minimal.

The business grew quickly.

But Kathryn soon discovered a hidden problem.

Boundaries.

Appointments ran late.

Travel times expanded.

Schedules shifted unexpectedly.

A day that looked manageable on paper often stretched far longer than expected.

Eventually she realized she wasn't building a scalable business.

She was creating a demanding job.

That realization pushed her toward opening a physical clinic location.

Finding Guidance Through Clinical Rainmaker

Shortly after launching Elite Physical Therapy, Kathryn joined Clinical Rainmaker.

Like many clinicians, she entered business ownership with extensive clinical expertise but very little formal business training.

At the time she was:

  • Using Word documents for documentation
  • Managing scheduling through her personal phone
  • Building systems from scratch
  • Learning through trial and error

Clinical Rainmaker provided structure, mentorship, accountability, and a community of practice owners facing similar challenges.

Most importantly, it shortened the learning curve.

Instead of reinventing every system herself, Kathryn gained access to proven frameworks and experienced guidance.

The Pricing Lesson Every Practice Owner Must Learn

One of Kathryn's biggest breakthroughs involved pricing.

When she first launched her practice, she charged only $80 per session.

Eventually she raised prices to $120.

Today, Elite Physical Therapy charges significantly more because Kathryn learned something many clinicians struggle to accept:

Your expertise has value.

Your time has value.

Your outcomes have value.

And charging appropriately allows you to build a stronger business that benefits both patients and staff.

The lesson wasn't simply about making more money.

It was about creating sustainability.

Higher pricing allowed her to invest in her team, improve the patient experience, and create better career opportunities for clinicians.

Building a Practice Around Family and Flexibility

Kathryn's vision extended beyond helping patients.

She wanted to build a workplace that clinicians actually wanted to work in.

Many physical therapists have no desire to own a business.

That doesn't mean they shouldn't have flexibility, autonomy, and fulfilling careers.

Today, Elite Physical Therapy emphasizes:

  • Family-first scheduling
  • Clinical autonomy
  • Flexible work arrangements
  • Professional growth opportunities
  • Collaborative care

The goal isn't maximizing patient volume.

The goal is creating an environment where both patients and clinicians thrive.

Learning How to Hire

One of the most valuable lessons Kathryn learned involved hiring.

Early on, she assumed everyone would approach the business with the same mindset and values she had.

That wasn't always the case.

Over time she discovered the importance of hiring intentionally and evaluating candidates based on cultural alignment rather than clinical skills alone.

She learned that the right team members accelerate growth.

The wrong team members create friction.

Like many successful owners, she now believes hiring slowly and intentionally is one of the most important decisions a practice owner can make.

Finding the Right Niche

Elite Physical Therapy has evolved significantly over the years.

Initially, the practice offered a broad mix of services and attracted a diverse patient population.

Over time, Kathryn refined the clinic's focus.

Today, the practice works heavily with:

  • Active adults
  • Orthopedic patients
  • Pelvic health clients
  • Return-to-sport athletes
  • Chronic pain populations
  • Individuals focused on long-term strength and wellness

Rather than trying to be everything to everyone, Elite Physical Therapy has intentionally built a reputation around serving the populations it helps best.

The Power of Community and Mentorship

Another major theme throughout Kathryn's story is mentorship.

She believes successful people consistently seek coaching, guidance, and accountability.

That belief led her from Clinical Rainmaker into the PT Biz Mastermind community, where she connected with practice owners from across the country.

The value wasn't simply learning new strategies.

It was gaining perspective.

Being surrounded by other owners helped her make better decisions, avoid common mistakes, and continue growing as a leader.

The Future of Elite Physical Therapy

Today, Elite Physical Therapy has grown far beyond its concierge beginnings.

The practice now includes multiple clinicians and a clear vision for future growth.

Rather than pursuing rapid expansion or franchising, Kathryn's focus is building a larger facility where exceptional clinicians can work together while maintaining the culture and patient experience that define the practice.

Her mission remains simple:

Help people return to the activities they love.

Whether those people are patients or clinicians.

Key Lessons From Kathryn's Journey

Burnout Often Comes From Misalignment

Many clinicians don't dislike physical therapy.

They dislike practicing in environments that prevent them from delivering great care.

Boundaries Matter

Without systems and boundaries, business ownership can quickly become overwhelming.

Charge What You're Worth

Appropriate pricing creates sustainability for patients, staff, and the business.

Hiring Changes Everything

The right people make growth easier.

The wrong people make it harder.

Mentorship Accelerates Growth

Learning from people who have already solved similar problems shortens the path to success.

Build Around Your Values

Your values should guide every decision, from hiring to patient experience to long-term growth.
